(1) A letter or affidavit signed by the

applicantn requesting the extension, including complete P.O. box mailing

address and telephone number;

 

(2) A copy of a valid identity document,

such as passport biographic page (with photo, date of birth and expiration

date);

 

(3) A copy of the I-94 (front and back);

(4) A copy of the umbrella permit (if

applicant has one); and

 

(5) A letter from the employer verifying

his or her intention to continue the applicant’s employment (if currently

employed).
